Hi - I posted early yesterday (after a 3 am finish) seeking help with 
stitching in PTGUI for images shot with a Canon 15mm fisheye, and 
hoping for a late submission. I fixed it all and made it with just an 
hour to spare to the deadline. Here is the washup....

Here is what occurred:

Many offers of help were made - thanks to you all, you know who you 
are.

I had auto-generated control points in PTGUI - this was not needed 
and was a cause of the stitch problems. I redid the pano from the 
start, using only a few control points and this worked well.

I posted that I have a Canon 5D - this was an error (I was tired 
after just 3 hours sleep, and must have been thinking wishfully!). I 
actually have a Canon 10D, and it seems that PTGUI and the Canon auto-
orientation feature don't get on well. Advice recieved (thanks Peter 
N!) shows that this should be disabled.

I reduced the complexity of the stitch by using only 2 rows of 10 
shots each and didn't need a central row, as there was just enough 
overlap. I didn't need a zenith (although I ended up using one), and 
I did graft in a nadir (shakey from slow shutter speed but OK 
nevertheless).

Summary for Canon 10D and Canon 15 mm fisheye for good results:

. turn off auto orientation; 
. 2 rows of 10 shots;
. 45 degrees up, 45 degrees down;
. zenith not actually needed, nadir used to hide tripod;
. don't autogenerate control points.

Result - my best one yet - hope you agree!
